Step 1: Your prescription

A. If your pharmacy already has your prescription for Levlen, Tri-Levlen or Levlite, please provide us with your pharmacy's name, phone and fax number.
Our pharmacy will contact it to transfer your prescription.

B. If you have a paper prescription, please send it to:

C. Currently on another brand? Ask your health care provider which of the brands in our program is right for you.

  • Once you receive your paper prescription, simply follow the directions provided above (B).
  • Or, our pharmacy can fax your health care provider for your prescription.

Step 2: Enroll in one of three quick and easy ways:

  • Complete and submit electronic enrollment form.
  • Print enrollment form, complete, and submit by mail.
  • Call 1.866.376.6523

Step 3: Now, just figure out what to do with the $$ you save.

  • Once we have your enrollment information, it will take a few days to process. You will need a prescription for one of our brands in the program.When your account has been established and our pharmacy has your prescription information, your pills will ship in about a week. (Standard shipping on your prescription is free; overnight delivery is an additional $19.
